Folliculometry - ultrasound monitoring of the ovaries during one or several menstrual cycles. The method of folliculometry is used to dynamically assess the state of the ovaries and endometrium. It allows to track the maturation of follicles, to identify the dominant follicle, the time of ovulation and the formation of the corpus luteum during one cycle. The state of the endometrium, its dimensions for subsequent attachment and implantation of the fertilized egg are also evaluated.

What do we conduct folliculometry for?

  • determination of the most favorable day for conception.
  • assessment of ovarian health
  • diagnosis of disorders of folliculogenesis and menstrual cycle.
  • control of hormonal levels.
  • identification of pathologies of the female genital organs (cystic formations, fibroids, etc.).
  • control of the probability of conceiving twins in a situation if there have already been multiple pregnancies in history.
  • assessment of endometrial status for successful implantation of a fertilized egg.
  • control diagnosis during ovulation stimulation and in vitro fertilization.
  • evaluation of the results of taking the drugs during treatment.

When is folliculometry conducted?

  • infertility (inability to become pregnant for more than a year, provided that the couple has a regular sex life and does not use any methods of contraception);
  • hormonal disorders (anovulation, polycystic ovary syndrome, luteal phase deficiency, and others);
  • when stimulating ovulation in polycystic ovary syndrome and other conditions;
  • in preparation for intrauterine insemination (artificial insemination with a sperm of a husband or donor);
  • during IVF for the selection of eggs.

Doppler sonography is the study of blood flow in the fetal, umbilical and uterine vessels. According to the results of Doppler ultrasound, the state of the uteroplacental blood flow is determined, which makes it possible to assess the prenatal state of the child (whether the child receives enough oxygen and nutrients, does not lag behind in development).

Indications for Doppler:

  • on the mother’s side (diabetes mellitus, hypertension, kidney disease, vascular system, gestosis)
  • on the part of the fetus (multiple pregnancy, low water, intrauterine growth retardation, premature maturation of the placenta)
  • burdened history (information received from a woman about previous pregnancies - fetal fetal death, intrauterine growth retardation, preeclampsia).

Ultrasound screening during pregnancy can detect 80-98% of fetuses with certain anatomical defects. The method involves at least 3 studies during pregnancy: at 11-12 weeks, at 18-20 and 30-32 weeks.

 

Ultrasound screening for a period of 11-12 weeks:

• allows to identify serious defects in fetal development: Down syndrome, omphalocele, gastroschysi, cystic hygroma, etc.

• the main feature in the calculation of risks is the width of the cervical transparency, or collar space (estimated subcutaneous fluid located on the back of the fetal neck).

• blood flow in the venous duct is also evaluated (disruption of its waveform is present in 80% of fetuses with Down syndrome) and nasal bone (which is not detected by ultrasound in 75% of fetuses with Down syndrome).

 

Ultrasound screening at weeks 18-20 and 30-32:

• at a period of 18-20 weeks, the study makes it possible to identify most of the gross abnormalities in the anatomical structure of the fetus (brain and spinal cord defects, gross heart and kidney defects, defects in the formation of limbs, etc.). Many of them are not subject to therapy and are a medical indication for induced abortion.

• at stage 30-32 weeks, ultrasound screening of the fetus can reveal anomalies of minor anatomical severity (heart defects, narrowing of the urinary tract, etc.). Most of them are subject to surgical treatment after childbirth.

At the same time, we recommend to undergo dopplerography, which allows to diagnose violations of placental blood flow and prevent fetal hypoxia and placental insufficiency.
